I am a registered occupational therapist providing mental health and neurodiversity-affirming services to adults in Manitoba. I operate a private practice under the name Tanya Neuert Therapy. What Information I Collect

As part of your care, I may collect: name, contact details, date of birth; medical, psychosocial, and occupational history; session notes, assessments, and treatment plans; information from other providers or fundors (with consent); payment and billing information (if applicable).

How I Collect Information

Your information may be collected through: intake and consent forms; virtual or in-person sessions; phone, email, or other communication (with consent); reports or referrals from other professionals (with consent).

Why I Collect It

Information is collected to: provide occupational therapy services; document treatment and clinical decisions; coordinate care (with consent); process billing or funding; meet legal and regulatory requirements.

How I Use and Disclose Information

Your information is used only for the purposes listed above, shared with others only with your informed, written consent, and disclosed without consent only when required by law (e.g., risk of serious harm, legal order).

How I Store and Protect Information

All records are stored in a secure, PHIA-compliant system (Jane App). Devices are password-protected and encrypted. Only I (Tanya Neuert) access your full health record. Secure platforms are used for communication when possible.

Your Rights

Under Manitoba’s Personal Health Information Act (PHIA), you have the right to: access and review your records; request corrections to inaccurate or incomplete information; withdraw or change your consent at any time; raise concerns or ask questions about your privacy. To make a request, contact me directly.

Retention of Records

Client records are retained for a minimum of 10 years from the last date of contact, in line with regulatory standards.
